Effective date: September 5, 2026
Our commitment
We are required by law to maintain the privacy and security of your protected health information, to provide you with this notice of our legal duties and privacy practices with respect to that information, to notify you following a breach of unsecured protected health information, and to follow the terms of the notice currently in effect.
How we may use and disclose your information
Treatment: We may use your health information to provide, coordinate, or manage your care. For example, we may share your exam findings with a physical therapist or physician we refer you to.
Payment: We may use and disclose your health information to bill and collect payment for the care you receive. For example, we may provide a receipt or treatment record that you have asked us to send to a third party on your behalf.
Health care operations: We may use your information for quality assessment, staff review, licensing, training, and other administrative activities necessary to run the practice.
Appointment reminders and treatment options: We may contact you to remind you of an appointment, or to tell you about treatment alternatives or health-related benefits and services that may interest you.
Uses and disclosures permitted or required without your authorization
State and federal law permit or require us to disclose health information without your authorization in certain narrow circumstances. These include public health activities; reporting suspected abuse, neglect, or domestic violence; health oversight activities such as audits and investigations; judicial and administrative proceedings in response to a court order or lawful subpoena; law enforcement purposes; workers' compensation claims; coroners, medical examiners, and funeral directors; organ and tissue donation; approved research; and to avert a serious and imminent threat to health or safety.
Where a state law is more protective of your information than HIPAA, we follow the more protective law.
Disclosures that require your written authorization
Most uses and disclosures of psychotherapy notes, any use or disclosure for marketing purposes, and any sale of your health information require your written authorization.
Any other use or disclosure not described in this notice will be made only with your written authorization. You may revoke that authorization in writing at any time, except to the extent we have already acted in reliance on it.
Your rights regarding your health information
Access and copies: You have the right to inspect and obtain a copy of your health records, including an electronic copy where we maintain them electronically. We may charge a reasonable, cost-based fee.
Amendment: You have the right to ask us to correct health information you believe is incorrect or incomplete. We may deny the request, and if we do we will tell you why in writing.
Accounting of disclosures: You have the right to receive a list of certain disclosures we have made of your health information.
Restrictions: You have the right to ask us to limit how we use or disclose your health information. We are not required to agree to most requests. We must agree, however, if you ask us not to disclose information to a health plan about care you paid for in full and out of pocket.
Confidential communications: You have the right to ask that we contact you at a specific phone number or address, and we will accommodate reasonable requests.
Breach notification: You have the right to be notified if a breach of your unsecured protected health information occurs.
Paper copy: You have the right to receive a paper copy of this notice on request, even if you agreed to receive it electronically.
